Survodutide
Survodutide (BI 456906) is a dual agonist targeting both the GLP-1 and glucagon receptors. This dual mechanism provides powerful weight loss through GLP-1-mediated appetite suppression and glucagon-driven increases in energy expenditure and hepatic fat oxidation. Clinical trials have demonstrated significant weight loss (up to 18-20% body weight) and remarkable improvements in metabolic-associated steatohepatitis (MASH), including fibrosis resolution. The glucagon component differentiates survodutide from pure GLP-1 agonists by actively promoting fat burning in the liver, making it particularly promising for patients with fatty liver disease alongside obesity.

Dosing Protocol
Titrate up from 600 mcg to 4.5 mg weekly. Starting dose is 600 mcg. Same day every week. Inject subcutaneously.
Reconstitution
Add 1 mL bacteriostatic water to a 6 mg vial (0.06 mg per unit).
Timing
Once weekly on the same day each week. Can be taken at any time of day, with or without food.
